The Colorado State Veterans Nursing Home at Fitzsimons, located on 15 acres of the former Fitzsimons Army Medical Center, is a state-of-the-art nursing home serving veterans and their spouses. We are committed to caring for the whole person by addressing the personal and social needs of each resident, as well as their physical and medical requirements. All staff members of Fitzsimons are committed to providing a life of quality and dignity for every resident. We offer free employee parking and easy access to RTD routes.
 
Fitzsimons Veterans Home is looking for  part-time and full-time LPN/Health Care Techicians to join our Nursing team.  There are numerous vacancies and the posting will remain open until filled.     

Description of Job


Plan and coordinate basic nursing care and provide skilled level of nursing to residents by assigning tasks to nursing assistants, administering medications and treatments as prescribed by physician.

Nursing Services:  Administers specialized and routine nursing services such as medication, treatments, etc.

Record keeping:  Documents assessments and care provided in the medical record and other appropriate documents i.d., MDS and Care Plans 
 
Nursing Care:  Utilizing nursing process, develops nursing care plans to address the psychosocial and spiritual needs of the residents. Supervise the clinical needs of the resident by observing their condition through rounds, determine residents' whereabouts and identify and remove any potential hazards.
 
Assessments:  Assesses the resident upon admission and throughout the clinical sty when critical incidents, emergencies, change in condition or acute illness and behavioral changes to identify nursing interventions and/or physician referral.

Compliance:  Review and audits specific care activities to determine compliance with policies standards of care as part of a department quality assurance activity.

Work Leader:  Work leader for Client Care Aide staff and makes staff assignments when serving as charge nurse on nursing unit.
 
Communications:  Provides and communicates information to residents and families related to residents' condition, change of condition and needs for services not available at facility. 
 
Skills, Knowledge, and Abilities:

  • Skill in initiating, performing and completing assigned duties in providing care to residents.
  • Skill in preparing and administering prescribed medications (oral, topical, subcutaneous, intramuscular and/or intravenous) performs treatments according to established policies/procedures.
  • Knowledge and ability to appropriately carry out assigned resident care based on the residents' conditions; to use judgment in selecting the appropriate order and sequence of procedures and treatments; and to accurately recognize, report and record relevant resident information.
  • Knowledge and ability to recognize urgent or emergent resident care situations, seek assistance of the RN and/or MD/DON, and initiate appropriate emergency interventions as directed.
  • Ability to observe, identify and respond to the resident's needs for care, including medication, equipment-assisted care and patient/family education.
  • Superior verbal, listening and written communication skills with all health care providers, residents, families and general public.
  • Demonstrated ability to display professional conduct at all times when dealing with fellow staff members, patients, families and the general public.
  • Demonstrated ability to maintain confidentiality at all times.

Minimum Qualifications, Substitutions, Conditions of Employment & Appeal Rights


Minimum Qualifications:

  • Education:  A high school diploma or GED;  ~And~
  • License:  Must possess valid Licensed Practical Nurse License from the State of Colorado or any state participating in the Nurse Licensure Compact;  ~And~
  • Work Experience:  At least two (2) years of professional experience as a LPN.  

Preferred qualifications:  

  • Experience in long term care or assisted living facility. 

Substitution:

  • College course work appropriate to the assignment may substitute on a year-for-year basis for the required experience.

This position requires the following as conditions of employment:

  • Must pass a Colorado Bureau of Investigations and a Federal Bureau of Investigations background check as a condition of employment. Included in the background check is a current and/or previous employer check.
