Position Summary

The Aging & Disability Resource Center (ADRC) provides information and assistance, options counseling, and advocacy to the general public, specifically older adults, adults with disabilities and youth age 17 years, 6 months.  Services include:  information regarding community resources, inform and educate on long term care options and connecting to programs and services, including publicly and privately funded options.

Work Hours:  8:00 a.m. - 4:30 p.m.

Essential Duties and Responsibilities

This job description reflects management's assignment of essential functions; it does not prescribe or restrict the tasks that may be assigned.
 
Determine eligibility for and develop service plans, coordinate and monitor services and provide on-going case management to adults receiving funding from AFCSP.
 
Provide information about programs, services (public and private) and public benefits, make referrals, and, when needed, assist with connecting to appropriate services.
 
Provide short-term case management according to ADRC policy.
 
Provide transition services to help youth who have a disability transition from school into the adult long term care system.
 
Provide advocacy, intervention, assessment, service coordination and other services as needed.
 
Provide assistance for and complete functional and financial eligibility determination for enrollment in the Long-Term Care Programs.
 
Complete annual protective placement reviews for persons who are elderly, physically disabled or intellectually disabled and are enrolled in a Long-Term Care program.
 
Participate in ADRC functions related to the general operation including trainings, staffings, meetings, resource development and marketing and outreach.
 
Demonstrate a commitment to county safety and risk management efforts.

Qualifications

Education and/or Experience
Bachelor's degree or Masters degree in social work, or in a health and human services related field, from a program accredited by, or pre-accreditation program of the Council on Social Work Education or a doctoral degree in social work and zero to six months of relevant prior experience.  Knowledge of long term care programs, such as COP, Family Care, IRIS, AFCSP, SSIE, and MA covered services. Proven experience in ability to add, subtract, multiply and divide in all units of measure, using whole numbers, common fractions and decimals as well as ability to compute rate, ratio and percent and to draw and interpret bar graphs.  Knowledge of Trauma Informed Care Principles is essential.

Provides specialized information and/or recommendations to others regarding an area of expertise.
 
Certificates, Licenses, Registrations
Valid Wisconsin drivers license.
Proof of minimum liability auto insurance coverage.
Wisconsin Social Worker License or certification - A temporary certificate will meet the requirements for initial employment when an individual meets all the qualifications for the permanent certificate except for passing the required examinations.  A permanent license/certificate is required for continued employment within nine (9) months of the date of hire; failure to attain said license or certification will be grounds for termination.  Any lapse in Wisconsin Social Work license/certification is cause for immediate termination. Or the equivalent - a Marriage and Family Therapist or a Professional Counselor license as defined by the Wisconsin Marriage and Family Therapy, Professional Counseling, and Social Work Examining Board.
