Description

Brunswick County is seeking a Public Health Nurse that performs counseling and care management duties to assist individuals and families through the programs administered by the North Carolina Division of Health and Human Services (NC DHHS), the North Carolina Division of Medicaid, and the Community Care of the Lower Cape Fear (CCLCF).  Work involves the supervision of Pregnancy Care Management (PCM) and Care Coordination for Children (CC4C) program staff providing services to children, pregnant women, and families in Brunswick County.  Further, this position shall oversee efforts towards outreach, population identification, assessment stratification, interventions and tasks along with collaboration with local community networks.  Provides care management that includes home visits, phone calls and practice visits for pediatric and adult patients identified from referrals and data lists. 

Hiring Range: $55,180-$71,734

Essential Duties

Must have excellent interpersonal, communication, motivational interviewing and negotiation skills.  Must have strong computer skills, as well as, strong organizational, and time management skills.  Employee must have ability to work independently and collaboratively.
 
Employee must be able to work with and relate to all types of people. Employee is also responsible for maintaining clear, concise records and files pertaining to assigned cases, as well as, plan and execute work effectively.  Employee must exercise independent judgment, discretion and initiative in completing assignments. Employee must also exercise considerable tact, courtesy and discretion in contact with program applicants and the general public. Reports to the Director of Nursing.

Qualifications

Bachelor of Science degree in Nursing with one year of Public Health nursing experience, or an Associate's degree in nursing and 2 years of nursing experience, one of which includes Public Health or an equivalent combination of education and experience.
 
CC4C/PCM program staff must demonstrate:
 
-Proficiency with the technologies required to perform care management functions-particularly as pertains to utilization of the Care Management Information System and Informatics System
 
-Motivational interviewing skills and knowledge of adult teaching and learning principles;
 
-Ability to effectively communicate with families and providers; and
 
-Critical thinking skills, clinical judgment and problem-solving abilities.
 
SPECIAL REQUIREMENTS
Must possess a current R.N. license from the North Carolina Board of Nursing.  Must possess a valid North Carolina driver's license. CPR Certification required.

Physical Requirements:  Must be physically able to operate a variety of machinery and equipment, including computers, calculators, copiers, facsimile machines, scales, sphygmomanometers, stethoscopes, etc. Must be able to exert up to 35 pounds of force occasionally, and/or up to 15 pounds of force frequently, and/or a negligible amount of force frequently or constantly to lift, carry, push, pull or otherwise move objects. Physical demand requirements are for Light to Medium Work.
