Position Summary
- This position is responsible for performing basic nursing activities in the course of care of residents in addition to supervising the activities of the Certified Nursing Attendants (CNA) and the Certified Medication Assistants (CMA).SUPERVISIONDirectly supervises employees including CNAs and CMAs while carrying out supervisory responsibilities in accordance with the organization's policies and applicable laws. Responsibilities include training employees; planning, assigning, and directing work; appraising performance; rewarding and disciplining employees; addressing complaints and resolving problems.SCHEDULEUnit: Memory or Medical CareDays: Varies per position

This job description reflects management's assignment of essential functions; it does not prescribe or restrict the tasks that may be assigned.
Utilize the nursing process: collect and record data; participate in development, implementation and evaluation of the residents plan of care and update CNA care cards and flow sheets.
Participate in quality improvement activities.
Observe, report and document all physical and/or behavioral changes in residents and subsequently share all pertinent clinical information with immediate supervisor.
Assure continuity of care through shift-to-shift report of the entire direct care team for the assigned neighborhood.
Follow physicians' orders and treatments, as directed.
Assist in the processing of physician orders, calls and accept verbal or telephone orders and transcribe according to LHCC policy and procedure.
Review physician orders and monthly medication and treatment records every thirty days.
Administer and document resident medications prescribed for routine administration, as well as "as needed" medications with registered nurse consultation.
Assume a leadership role during fire drills or emergency situations.
Monitor direct care staff to ensure that resident's rights are maintained.
Interact directly with residents, their significant others and the representatives of other agencies on an ongoing basis.
Assume responsibility for the whereabouts of all residents in assigned area(s).
Complete the Minimum Data Set.
Complete daily documentation that addresses who, what, when, where, why and how.
Report resident grievances, allegations of abuse or misappropriation of property to the management person in charge.
Review the CNA witness statements for thoroughness and completion during an investigation.
Assign duties and resident assignments to the CNAs and CMAs each day as well as follow up to ensure assignments have been completed. Review CNA documentation for accuracy on a daily basis.
Participate in the counseling and formal performance evaluations of the CNAs and CMAs, and when necessary, consult with the appropriate Nurse Manager for assistance with this process.
Participate in the orientation of licensed staff, CNAs and CMAs.
Recommend CNAs for promotions or to act as a preceptor for new staff.
Maintain personal professional competence through participation in nursing and continuing education opportunities.
Attend and participate in all in-service educational opportunities, as appropriate to the job classification.
Participate in the evaluation of supplies and monitor staff for cost effective supply utilization.
Request maintenance on all equipment, as required.
Participate on committees as well as attend and participate in nursing meetings.
Demonstrate a commitment to county safety and risk management efforts.Qualifications

High school plus Technical diploma in practical nursing and zero to six months of relevant prior experience. Experience in restorative nursing, long-term care, psychiatric nursing, medical nursing or geriatric nursing preferred. Proven experience in ability to calculate figures and amounts such as discounts, interest, proportions and percentages as well as ability to apply basic algebra and geometry concepts to calculate area, circumference, volume, and/or land measure. Proven experience with medical terminology.

Certificates, Licenses, Registrations and Other Special Requirements
Wisconsin Licensed Practical Nurse license through WI Department of Safety and Professional Services.
Criminal background check free of convictions of abuse of individuals or misappropriation of property while in a formal caretaking setting.
All staff of Lakeland Health Care Center may be called upon to respond to emergency situations assisting residents in their physical positioning.
If you are not available to work on the date this position starts, you will not be awarded the position.
All employees may be required to work overtime as determined by the department head.

While performing the duties of this job, the employee is regularly exposed to moving mechanical parts and fumes or airborne particles.
