AGING PROGRAM SPECIALIST II 

The Health and Human Services Agency, Aging & Independence Services (AIS) provides services to older adults, persons with disabilities and their family members, to help keep clients safely in their homes, promote healthy and vital living, and publicize positive contributions made by older adults and persons with disabilities.

As an Aging Program Specialist II (APS II), you may be responsible for assessing clients' social support needs. This includes explaining the available services and supports, enrolling clients into programs, establishing a customized service plan, making referrals for counseling, AIS programs and services, or community-based support services. You may also provide education to clients and their caregivers about the availability and eligibility of social supports and how to navigate the system to access services. Lastly, as an APS II, you may participate in community outreach programs by facilitating community meetings, giving presentations on a variety of topics, managing and coordinating volunteers, and representing AIS within the community.

Applicants to this classification may be utilized in one or more of the following areas: AIS case management programs, the Long Term Care Ombudsman Program, AIS Call Center, Community-based Care Transitions Program, Outreach and Education, Senior Volunteer Program, and Legacy Corps.

Minimum Requirements:

A bachelor's degree from an accredited U.S. college or university, or certified foreign studies equivalency in gerontology, psychology, social work or related behavioral or social science field of study; AND,

Two (2) years of professional social work experience working with senior and/or disabled citizens in adult protective services, in-home services, conservatorship, public guardian or similar programs performing duties related to case management, program development and community organization, community resource information and referral, or long term care.

Note: A master's degree in gerontology, social work or a similar social science degree can substitute for one (1) year of experience. Formal internships at the Master's level may substitute for the experience requirement on a year-for-year basis.

Ideal Candidate:

The ideal candidate will act as the Aging Program Specialist II (APS II) in one of the following key programs at Aging & Independence Services:

In the Community-based Care Transitions Program (CCTP), the APS II will act as a short term care coordinator and provide patients and their caregivers immediate and critical social support services, either by referral or direct provision of support services, to reduce the risk of an avoidable hospital readmission.

In the Multipurpose Senior Services Program (MSSP), Senior Options Advocacy Referral (SOAR) Program, and Linkages Program, the APS II conducts long-term care management for frail or elderly. This candidate conducts in-depth assessments and reassessments covering psychosocial, rehabilitation, and environmental concerns. This position also provides service coordination, ongoing monitoring, and evaluating of client costs to ensure cost containment.

The APS II in the Long-Term Care Ombudsman Program (LTCOP) is a resident rights advocate, supervises/trains ombudsman volunteers, and investigates reports of abuse and neglect of residents in long-term care facilities.

The Call Center is the centralized intake for AIS programs. The APS II in this role takes reports of elder and dependent adult abuse, complete referrals for various care management programs as well as the In-Home Supportive Services program. This candidate also provides assistance with community resources.

In Outreach & Education, Senior Volunteer Program or Caregiver Support Program, the APS II facilitates community meetings, gives presentations on a variety of topics, represents AIS at meetings in the community and reports back outcomes, coordinates volunteers, and organizes special events.
