The Outreach Coordinator performs a variety of community health promotion and outreach work in support of county health programs. Work involves considerable contact with agencies and clients to provide information and brochures, and using computers to design and develop promotional brochures and information.

Examples of Duties

Contacts and maintains working relationships with local health and human service agencies to provide information regarding Denton County Health Department programs and services.

Contacts potentially eligible clients to encourage participation in Children's Health Insurance Program (CHIP), Medicaid, WIC, Immunization, and any other programs and services which the family may be eligible for.

Assists families in acquiring and completing eligibility forms for appropriate programs and services for Denton County Health Department, and other agencies as well.

Contacts CHIP clients monthly to serve as a reminder to renew coverage.

Attends community health events, distributes health education information, and assists in coordinating public health efforts with Denton County Health Department Health Educator.

Utilizes computer software to design and develop brochures and flyers that will effectively promote public health.

Coordinates with CHIP providers to maximize utilization of CHIP services among potentially eligible families within Denton County.

Coordinates the work of interns and/or volunteers to maximize program effectiveness.

Manages grant budget to ensure optimal use of allocated funding.

Utilizes computerized data entry equipment and various word processing, spreadsheet, and/or file maintenance programs to enter, store, and/or retrieve information as requested or otherwise necessary.

Prepares and prints out various forms and reports.

Maintains various activity and client logs, submitting according to established procedures.

Conducts state assistance training for local health and human service agencies and school personnel partners.

Regular and punctual attendance is required.

Performs other related duties as required.

Minimum Qualifications

Bachelor's degree in related field and a minimum two years of experience in a related field; or an equivalent combination of education and related experience required.

Certified Health Education Specialist (CHES) preferred.

Possession of a valid driver's license.  Must be able to obtain a State of Texas driver's license within 90 days of employment.
